Lifefy policies are issued by The Savings Bank Mutual Life Insurance Company of Massachusetts (“SBLI”), Woburn, MA. NAIC #70435. Centrian Life Insurance is a registered trade name and Connecticut d/b/a of SBLI. Policies sold in the state of Connecticut will be under Centrian Life Insurance. Riders are available at an additional cost. Products and features may not be available in all states. Policy form series 18-P-SIT and 18-P-GIT. Rider form series 18-R-AXRDB, 18-R-AXDDB, 18-R-WOP and 18-R-GIAXDDB. SBLI is rated A (Excellent) by A.M. Best, A- (Strong) by Standard and Poor’s, and A+ by Better Business Bureau. 1 Approved claims paid within 48 hours (two business days) of receiving correctly completed paperwork. 2 Premiums change after the level term.  3 Varies by issue age.  4 Activation fees may apply. Complimentary only with Simplified Issue product.  5 Death benefit in the first few years is limited to a return of premium.  6 Full refund if policy is cancelled within the free-look period (20-30 days depending on state).
